Jennifer Provides initial psychosocial evaluations, ongoing psychosocial counseling of patients and families, addressed end of life issues, grief counseling, and follow up care with families after the loss of a loved one. Provided community resource needs identification, outreach, and referral.

Jennifer also provides direct casework service, financial assistance, long and short range goal planning with patients and families, case coordination with other disciplines and treatment professionals, liaison with grief counselors in community, local and state agencies, area funeral homes, and hospital staff involved with patient cases.
